  1. Submit the completed set of application forms together with the required documents stated under REQUIREMENTS:

  2. — Application for Admission

    — Certification and Admission Agreement

    — Recommendation Form

    — Affidavit of Guardianship

    — Health History and Updated Medical Certificate

  3. Pay for the application & testing fee at the cashier’s office.
  4. Present the receipt to the Admissions Office and get a copy of the applicant’s Admission Qualifying Test Permit.

Completed Application and Health Forms as well as the following should be prepared for submission to the Admissions Office:

  1. Certified copies of academic records for at least the past two years. If written in another language, an English translation must be provided.
     
  2. Two letters of recommendation from the former school, one from the Principal or Guidance Counselor, and one from a teacher, preferably the homeroom adviser. It is necessary to use the recommendation forms provided in this booklet.
     
  3. Copy of birth certificate or passport showing the applicant’s date of birth and photograph.
     
  4. Copy of immunization record.
     
  5. Photos: 2 copies (2" x 2") 2 copies (1" x 1")
     
  6. If school fees are to be paid by an employer, a letter on the employer’ letterhead stating this.
